SPMS Athletics
ATHLETIC MEDICAL EXAMINATION FORM

This MUST be completed by a physician before a student can compete in a game.  The fall season will officially begin in approximately 3 weeks.  ALL students who are planning on competing in any of the school athletic teams MUST have a physical.  The physical is valid for multiple seasons.  Forms will be available in the office and on the web.
*No exceptions to this rule
Athletic Eligibility
Each student must:
Maintain a 2.50 grade point average (‘C’) or higher in all classes they are enrolled in.  This requirement is rechecked each quarter.  Students who receive one or more grades lower than a ‘C’, will be removed from the team until the next grading period.  Students that are academically ineligible are not allowed to practice or participate in games.  Note:
grades are checked at the Quarter,  progress reports are not official grading periods for the purpose of athletic eligibility. 
Attend all classes on game day.  Our expectation is that our athletes will attend all their classes all the time.

Welcome to the SPMS athletic website.  The 2011-2012
athletics program is in full motion.  As for 6th grade Intramural sports (co-ed) , all information about the sports being offered will be shared with them during their PE classes or you can click on the link above for tentative dates. 
Some of the requirements to participate in sports here at SPMS are: maintaining a “C” or better in all classes, if you make a 7th or 8th grade team a physical examination from your doctor, and a parental permission slip to travel.  Unfortunately, 6th graders may only participate in intramural sports at SPMS.  If your child anticipates trying out for a 7th or 8th grade team this year, it might be wise to schedule a physical examination prior to the season starting. But as a reminder, just because you complete a physical, there is no guarantee that you will make the 7th or 8th grade team.  Once you have completed and submitted your physical form to Mr.Adanto Room 203 (Athletic Coordinator) it will be valid for the remainder of the school year.  If you are unable to schedule an appointment prior to the first league game, you may email Mr.Adanto and he has a contact of a local doctor in South Pasadena who also conducts physicals for SPHS sports teams at a very low cost. 
Given the current economic situation for public education our sports program is fortunate enough to have a very supportive Booster Club (Ms. Kate Gibson/President) http://booster.homestead.com .  If your child anticipates playing on a team and makes it, I would strongly encourage you to join our booster club so that the sports program at SPMS may continue as it has. You can also find membership forms for the Booster Club when visiting their website.   I look forward to an amazing year and look forward to meeting all athletes as the year progresses.
